Planning ahead ensures a smooth and stress-free hospital experience. If your visit is not an emergency, take the time to prepare by following these steps:
Bring valid IDs with personal details (e.g., current address, next of kin, employer).
Keep a list of your medical history, including medications and records, ready.
Bring your healthcare cards or insurance coverage forms, as well as any special form(s) or pre-authorization documents such as Letter of Guarantee (LOG) or Letter of Authority (LOA).
If you are married, bring information about your spouse's insurance coverage to ensure that you avail yourself of full insurance benefits as his/her dependent.
Please submit the necessary requirements and forms to avail of PhilHealth deductions from your final bill. Our PhilHealth Dept. Officer will guide you through the process and explain the details thoroughly. To ensure your hospital bill is adjusted promptly, please submit the completed and properly filled-out PhilHealth eligibility and claim documents “before your discharge.” To avoid delays, you or a family member may begin working on these documents immediately upon admission.
